- Select Performance Ratings+ from the top navigation.
- Click Formulas.
- Click Create formula.
- Enter a name for the formula in the Formula name field.
- Under Manager review, click Add questions to include manager review questions.
- Select each question to include from the list.
- Enter a weight percentage for each selected manager review question.
- Under Self review, click Add questions to include self review questions.
- Select each question to include from the list.
- Enter a weight percentage for each selected self review question.

- Select Delete.
- Confirm the deletion in the dialog.
If Something Goes Wrong
| Issue | Check | Fix |
|---|---|---|
| Weights do not total 100% | Review the percentage entered for each question | Adjust individual question weights until the total reaches exactly 100% |
| No questions appear in the question picker | Confirm review question templates exist | Create or publish question templates before building the formula |
| Save button is unavailable | Check that the formula has a name and at least one question added | Add a formula name and at least one weighted question |
| Cannot edit formula linked to an active cycle | Check whether any review in the cycle has been submitted | If no reviews are submitted, edit is still allowed; otherwise create a new formula |
