I received a 1004 error when opening my engagement survey link

When you open an engagement survey link, a 1004 error appears instead of the survey. This article covers how to diagnose and resolve that error.

Diagnose

Check: Survey was recently launched → Look at the survey launch time in your calendar invite or the email you received. → If the survey launched within the last few minutes: wait 5 minutes and retry the link. → If the survey launched more than 10 minutes ago and the error persists: continue to the Fix section.

Fix

If the survey launched recently:

  1. Wait 5 minutes after the survey launch time.
  2. Open the survey link again.

If the error persists after waiting:

  1. Note the exact error message text and the time you first saw it.
  2. Copy your survey link URL.
  3. Contact 15Five Support with the error details and link.

What Resolution Looks Like

After a successful retry, the survey loads and displays the first question. No error banner appears.

If It Still Fails

Contact 15Five Support directly. Provide the following when you reach out:

  • The survey link URL
  • The exact error message displayed
  • The date and time you first encountered the error
  • Your account email address
