An answer template defines the opinion-scale ratings available on opinion-scale questions in a review question template. Use this process to replace 15Five's default scale — Strongly Disagree, Disagree, Neutral, Agree, Strongly Agree — with a custom set of ratings.
Before You Begin
- You must have Review admin access to create answer templates.
- Answer templates are created before or during question template setup, not inside an active review cycle.
Steps
- Navigate to Reviews in the left sidebar.
- Click Settings.
- Select Answer Templates.
- Click Create Answer Template.
- Enter a name for the template in the Template Name field.
- Click Add Rating to add the first rating label.
- Type the label text for that rating.
- Repeat steps 6–7 for each additional rating in the scale.
- Drag rating rows to reorder them if needed.
- Click Save.
If Something Goes Wrong

| Template not appearing in question template setup | Confirm the template was saved without errors | Reopen the template and click Save again |
| Cannot delete a rating label | Check whether the template is already applied to a question template | Remove the template from any question templates before editing |
