Creating a Lifecycle Survey from scratch lets you build a fully custom survey without starting from a pre-built template.
Before You Begin
- You must have Account Admin access to the Lifecycle Surveys dashboard.
- This feature requires the Total Platform, Legacy Perform, or Engage pricing package.
Steps
- Click Surveys in the left-hand navigation, then select Lifecycle.
- Click Create survey underneath the pre-built templates.
- Enter a name for your survey, then click Continue.
- Choose Manual or Automatic (based on hire date) as the trigger type, then click Continue.
- Select who will participate: all employees, specific groups, or a refined audience with exclusions.
- Click Add new employee question or Add new manager question to add opinion-scale or text questions.
- For each question, set whether it is required or optional.
- Set how long the survey stays open after sending. The default is 14 days.
- Select whether managers can see their team's responses.
- Review all details on the Summary tab, then click Save survey.
> Automatic surveys send around 10 AM ET on their scheduled launch date. Participants receive an email notification from surveys-cloud104@15five.com. For manual surveys, you must trigger the survey separately after saving.
> When participants take the survey, they see a disclaimer stating whether their responses are visible to their manager.
If Something Goes Wrong
