Admins use this article to create a Lifecycle Survey from a pre-built 15Five template. When complete, the survey appears in your active Lifecycle Surveys list and is ready to trigger.
Before You Begin
- You must have Account Admin access to the Lifecycle Surveys dashboard.
Steps
- Go to Surveys > Lifecycle in the left-hand navigation.
- Click Customize under the template type you want to use.
- Update the survey name if needed, then click Continue.
- The name is visible to survey participants.
- Select Manual or Automatic (based on hire date) as the trigger type.
- Click Continue.
- Select the survey audience:
- All employees — includes everyone in your account - Specific groups — limits the survey to selected groups - Exclude employees — removes specific individuals from the audience
- Click Continue.
- Review the pre-populated questions on the Questions tab.
- To remove a question, click the trashcan icon to its right.
- To reorder a question, drag it using the 6-dot icon to its left.
- To add a question, click Add new employee question or Add new manager question.
- Set the number of days the survey stays open after it is sent.
- The default is 14 days.
- Select whether managers can see their team's responses.
- Note: Participants see a disclaimer during the survey stating whether their manager has visibility.
- Review all details on the Summary tab.
- Click Save survey.
The survey appears in your active Lifecycle Surveys list. Automatic surveys send at approximately 10 AM ET on the scheduled launch date. Participants receive an email notification from surveys-cloud104@15five.com.
If Something Goes Wrong

| Automatic survey does not send on the scheduled date | Verify the trigger type is set to Automatic and the hire date data is present for all participants | Check employee hire date records in People settings and confirm trigger configuration |
