Add question bank questions to Check-ins

Use this article to add a question from the 15Five question bank to your organization's Check-ins. When complete, the question appears on Check-ins for the audience and schedule you configure.

Before You Begin

  • You must have access to Check-ins and the Manage questions page. Account admins can create questions for anyone; Group admins can create questions for their group members; Managers can create questions for people in their reporting hierarchy. See for full permission details.

Steps

  1. Click the Settings gear in the bottom-left corner of your 15Five account.
  2. Go to Features Overview > Question Queue > Configure.
  3. Click the Question bank tab in the top navigation bar.
  4. Use the search bar and filters to find the question you want:

- Filter by category — returns questions aligned to topics such as culture, engagement, or recognition - Filter by question type — returns only text questions or only metric questions > Note: Text questions are answered with written responses and appear in the Access the Check-ins Questions Dashboard. Metric questions (Yes/No, Numerical, Rating scale) appear in the Access the Check-ins Polls Dashboard and Download Question Data and View Check-in metric trends over time (Trends Dashboard).

  1. Click Set up next to the question you want to add.
  2. Select the question audience:

- Company-wide — asked to everyone in the organization - Group — asked to members of the groups you select - Person — asked to specific individuals

  1. Configure the context settings by toggling which details appear below the question:

- Author — shows who created the question - Frequency — shows how often the question appears - Audience — shows who the question is directed to

  1. Select how often the question appears:

- Show in every Check-in — appears on all Check-ins every reporting period - This is a one-time question — choose to add it for the current or following reporting period - Schedule this question — set a recurring cadence: weekly, bi-weekly, monthly, or quarterly

  1. Select question settings:

- Required — respondents must answer before submitting their Check-in - Allow multiple answers — respondents can submit more than one answer (text questions only) - Include an optional written answer — respondents can add a qualitative answer alongside a metric response (metric questions only) - Share answers on Slack — enter a Slack channel name to post all answers there (only visible if your organization has the 15Five–Slack integration enabled)

  1. Click Create new question.

The question now appears in the question list and will display on Check-ins for the configured audience at the next applicable reporting period.

If Something Goes Wrong

Issue Check Fix
Set up button is missing next to a question Confirm you have the Manage questions permission for the intended audience Contact your Account admin to grant the required permission
Question does not appear on Check-ins after creation Confirm the question's schedule and audience match the affected users' Check-in period Open the question's settings and verify the audience and frequency; adjust if needed
Share answers on Slack option does not appear Confirm the 15Five–Slack integration is enabled for your account Contact your Account admin to enable the Slack integration before configuring this setting
403 error when attempting to access the question bank You do not have a role that grants question creation access See and contact your Account admin
