Engagement Survey Types in 15Five

When you create an engagement campaign in 15Five, you select one or more surveys to include. All selected survey statements are combined into a single response experience for employees.

Feature Structure

15Five offers four categories of engagement surveys:

  • Core Engagement Surveys — Pre-built, research-backed surveys measuring engagement, manager effectiveness, and employee loyalty
  • Driver Surveys — Targeted add-ons covering specific engagement drivers
  • Topic-Based Surveys — Pre-built surveys covering discrete workplace topics (benefits, burnout, retention, inclusion)
  • Custom Surveys — Surveys built from scratch by Account Admins

Core Engagement Surveys

Total Engagement (Recommended)

The most comprehensive option. Includes:

  • 7 core Engagement Score statements
  • Statements aligned to 17 engagement drivers
  • 3 Organizational Commitment statements
  • 8 Manager Effectiveness statements
  • eNPS question and open-response follow-up

Campaigns using Total Engagement or Engagement + Drivers have access to the Predictive Impact Model — Feature Overview in results.

Engagement Score

Includes only the 7 core statements used to calculate your organization's Engagement Score. Use this survey for a fast, focused pulse with no driver-level detail.

Manager Effectiveness

Collects anonymous employee feedback on direct managers across 15Five's core manager competencies. Results are available in engagement reporting and can be integrated into your organization's HR Outcomes Dashboard.

Driver Surveys

Driver Surveys let you add statements for specific engagement drivers to a campaign. Click the + icon next to any driver when setting up your campaign to include it.

To preview the statements in each driver survey, click the highlighted question link during campaign setup.

Topic-Based Surveys

Topic-Based Surveys cover discrete workplace topics. Available surveys:

  • Employee Benefits — Evaluates sentiment about current benefits and gathers improvement feedback
  • Burnout Assessment — Assesses burnout levels; uses a 5-point Likert scale
  • Working from Home — Identifies remote work challenges and gathers support feedback; uses a 5-point Likert scale
  • Employee Retention — Evaluates retention risk and commitment levels; uses a 5-point Likert scale
  • Diversity & Inclusion Progress — Gathers feedback on fairness, respect, and belonging; uses a 5-point Likert scale
Important: Include Topic-Based Surveys only when your organization has a plan to act on the results. See .

Custom Surveys

Account Admins can build surveys from scratch to cover topics not included in 15Five's pre-built options.

Key limitation: Custom Surveys do not generate an engagement score, driver data, or heatmap reporting. See .

Key Rules

  • A campaign can include more than one survey type; all selected statements appear as a single combined set for employees.
  • Core Engagement surveys and Engagement + Drivers surveys unlock access to the Predictive Impact Model in results.
  • Custom Surveys cannot be combined with core survey reporting to generate engagement scores or driver data.
  • Topic-Based Surveys require an organizational action plan before inclusion; including them without follow-through erodes employee trust.
